ATHLETES---

Physicals will be held on Tuesdays when the team doctor is in the district. Each Tuesday there will be a different team scheduled, see document below.

-Parents and Athletes must complete sections 1-6 of the PIAA physical form in order for the doctor to complete the physical.

-Physical packets can be found in the Middle and High School offices, the trainers office, or click here, Sports Physical Comprehensive Initial Pre-Participation Evaluation (CIPPE) and Allegheny Health Network AHN Addendum to CIPPE Form 

-If your child has participated in a fall sport, please only complete section 8 of the physical form.

-You may also choose to get your physical at your family physician, but you must use the same physical form.
