Athletic Physical Information
Cost is $15.00 when completed by the school physician. Cash payments are accepted as well as checks made payable to Northwestern High School. Dates are announced prior to each season.
Physical Packets are also available in the Middle and High School offices or the trainers office.
You may also choose to get your physical at your family physician, but you MUST use the physical form above.
