Parents and Athletes must complete sections 1-5 of the PIAA physical form in order f the doctor to complete the physical. 

Physical Packets can be found in the High School office, the athletic trainer's office or at https://www.nwsd.org/athle 

If you have participated in a Fall or Winter sport, please only complete section 7 of physical form (You will not need another physical). 

You may also choose to get your physical at your family physician, but you must use the same physical form.
