Volunteer Checklist (Policy 916 AR-4)-Clearances needed are listed below

• Act 34 PA State Police Criminal History Record Check

• Act 151 PA Child Abuse History Clearance Procedures

• Act 114 FBI Fingerprint Check Application Procedures - MUST be completed by all Volunteer Coaches and Overnight Chaperones

  ***Effective January 2023, volunteers who are renewing or obtaining a new FBI clearance must use the DHS (Department of Human Services) service code. The service code is 1KG6ZJ.  Questions can be directed to DHS directly at 877-371-5422 or email RA-PWCPSL Questions

Volunteer Affirmation Statement (please complete this form if you planning to volunteer as a single-day field trip chaperone, dance chaperone, etc. This form is NOT applicable to volunteer coaches or overnight chaperones)

*The Affirmation Statement replaces the fingerprint check for those who have lived in PA for ten (10) consecutive years.
